HCBS- DD waiver offers Residential services for individuals with intellectual and developmental disabilities (IDD) encompass a range of supports and accommodations provided in a residential setting to promote their well-being, independence, and quality of life. These services entail residing in a household overseen by a host home provider or A Family member who delivers round-the-clock care, addressing their daily necessities and facilitating their integration into community activities. Daily necessities include the following: Daily supervision and training help with Personal Hygiene, Medication Administration, Laundry, Behaviors, Coping Skills, Community integration and much more!
Children's extensive supports for IDD aim to provide comprehensive, multidisciplinary support to children and their families from early childhood through adolescence, with the goal of maximizing the child's potential, promoting independence, and enhancing their quality of life. These services emphasize a family-centered approach that recognizes the unique strengths, needs, and goals of each child and their family, with a focus on fostering holistic development and inclusion in all aspects of life. Services we provide: Homemaker Basic/Enhanced, Respite (Coming Soon), Massage Therapy (Coming Soon) and much more.
Supported Living Services are available to assist persons with developmental disabilities to live more independently in the person's own home, apartment, family home, or rental unit. The HCBS-SLS waiver is designed to supplement existing natural supports and traditional community resources with targeted and cost-effective services and supports. Participants on this waiver do not require twenty-four (24) hour supervision on a continuous basis for services and supports offered on this waiver. Services we currently provide: Family care giver, Homemaker basic/enhanced, Respite, Massage Therapy, Supported Community Connector, and Prevocational services.
Enhanced and Basic Homemaker services for individuals with intellectual and developmental disabilities (IDD) Consist of the following for Basic Homemaker: Light housekeeping, Meal planning and prep, Changing bed sheets, Laundry, Dishwashing, Cleaning bathroom and Kitchen , Vacuuming and dusting and Sweeping and mopping. With Enhanced Homemaker services there are a few additional services: Emptying Trash, Home maintenance supervision, Individual support in monitoring diet and nutrition, Sorting and reading mail, Deep housekeeping, Completion of tasks assistance.
Prevocational services are designed to prepare individuals for future employment by developing their vocational skills, work habits, and social competencies. These services aim to enhance the employability and independence of individuals with IDD, enabling them to transition to competitive integrated employment or other meaningful vocational opportunities. These services consist of: Acquiring and maintaining work habits and work related skills, Learning how to interview, Communication skills, Time management, Problem solving, task completion, Teamwork, and more. With an end result of finding employment.
SCC provides opportunities for individuals with IDD to engage in social activities and recreational pursuits within their communities. This may include participation in clubs, groups, classes, sports teams, cultural events, and leisure activities tailored to their interests and preferences.
Respite care for individuals with Intellectual and Developmental Disabilities (IDD) is a vital service that provides temporary relief to caregivers while ensuring the safety and well-being of the individuals they care for.
Respite care offers both the caregiver and the individual receiving care a much-needed pause and a chance to experience a change of environment, providing a refreshing break from the monotony of daily routines.
Massage therapy can offer numerous benefits, promoting relaxation, sensory stimulation, and overall well-being. Tailored massage techniques can address specific needs and challenges faced by individuals with IDD, providing them with a therapeutic and enjoyable experience. Massage therapy can also be utilized to alleviate pain and promote overall relaxation, aiding in clearing the mind.

A host home provider is a small business owner with whom A Glimmer of Hope LLC (AGH)
contracts to provide a specific individual with room and board as well as other residential
services and supports.
If space allows, you may have three clients in your home. If you have a third bedroom you
plan to use, you can also do respite services instead of hosting 3 individuals, full time. You can also be a Respite care specific Host Home Provider, meaning you would only provide respite care and emergency respite services. Respite care services came range from several days to months. Each client must have his or her own room with a locking door. Clients are able to share of room if they (clients) both agree on sharing that room and it is written in their service plan.
Each individual who is admitted to our services receives a standardized assessment. That
assessment determines the level of service needs, and we pay Host Home Providers based
on the rates paid for that level of service by the State of Colorado. Levels of service range
for level 1 - level 6. Pay ranges from $2600 - 6000 per month for residential services and room and board.
It varies depending on whether or not we currently have a client who is requesting a new
home, or if a new client is moving to the area. We don’t rush into placements for the
benefit of everyone concerned. Once we receive a Referral for someone needing service, we respond with interest and wait for the client or client's family member to contact our Agency letting us know they would like to get acquainted and spend a little time with one of our Host Home Providers they are interested in before a contract is signed, and they are moved into your home. However, if your training certifications and insurance requirements have been met, you can offer to provide respite care for a Host Home Provider who would like a short term break. Homes that are wheelchair accessible and those that are willing and able to accept clients with behavioral challenges are always in demand. Also, you are not obligated to provide host home services for A Glimmer of Hope, LLC., only. There are other agencies available!
